FOOD MENU AND FULL SERVICE OPERATIONS REVIEWED. NO APPARENT ISSUES OBSERVED. THIS FACILITY WILL BE CATEGORIZED AS A 'MEDIUM RISK' FACILITY, OR CATEGORY 2. NEXT STEPS: (1) ONCE ALL ASPECTS OF THE FACILITY ARE COMPLETE AND FINALIZED, SCHEDULE A PRE-OPENING INSPECTION WITH THE HEALTH DEPARTMENT (2) DURING THE PRE-OPENING INSPECTION, THE DEPARTMENT WILL REVIEW ADMINISTRATIVE DOCUMENTS, SUCH AS FOOD TRAINING CERTIFICATES AND ILLNESS REPORTING AGREEMENTS; FURTHERMORE, THE FINAL MENU AND THE FACILITY AND EQUIPMENT WILL BE EVALUATED. (3) ONCE THE PRE-OPENING INSPECTION IS COMPLETE, THIS FACILITY WILL BE ELIGIBLE TO SIGN UP FOR THE ANNUAL FOOD LICENSE. THE ANNUAL FOOD LICENSE FEE FOR A MEDIUM RISK FOOD FACILITY IS $350.00 |
